Vascular The Hague

In the Netherlands, and in The Hague in particular, we see significant discrepancies in health outcomes between patients in different communities and between patients from different backgrounds and social statuses.

An important step forward will be better identifying the care for patients with a high risk of cardiovascular disease in order to be able to offer them proper care for outcomes that help both the patient and the care provider/care sector (quadruple aim) and to measurably improve these outcomes.

Analysis of routine data obtained from GPs, hospitals and Statistics Netherlands (CBS) will lead to better identification and risk stratification for the prevention of cardiovascular disease. This means identifying specific at-risk groups for things like panel management and risk prediction models, in order to reduce the discrepancies in health outcomes.
